Parade Entry Form Template

Fields You Need

Contact Information

  • Entry name (as it should appear in the lineup)
  • Primary contact person (first and last name)
  • Phone number (mobile preferred — used on parade day)
  • Email address
  • Mailing address (for invoices/correspondence)

Entry Details

  • Entry category: float, band, vehicle, walking group, equestrian, drill team, or other
  • Brief description (1–2 sentences for the announcer to read)
  • Number of participants walking/riding
  • Vehicle type and length (if applicable — for spacing)
  • Music: live band, recorded music, or none
  • Any special requirements (power, water, animals, slow-moving)

Organization

  • Organization or business name (if applicable)
  • Has this group participated before? (yes/no, which years)
  • How did you hear about the parade?

Agreements & Fees

  • Entry fee amount and payment method
  • Liability waiver acknowledgment (signed/dated)
  • Photo/video release consent
  • Agreement to follow parade rules and marshal instructions

Entry fees vary by category and should cover your per-entry costs without discouraging participation. Here's what's typical for community parades:
